Bowery, RG, Project Manager <br /> STATUS: APPROVED with conditions noted below. <br /> ADEQUATE AND NECESSARY ELEMENTS OF THE WORKPLAN fHealth_& <br /> _Safety Code (H&S) Section 25296.10(c)(3)1: <br /> • Overbore destruction of 70' screened MW-5 and replace with three <br /> other, shorter screened wells (EW-5d, EW-5s, VW-5) and, <br /> • Collect and analyze soil samples from EW-5d for TPHg (8015m), <br /> BTEX and MtBE and other Oxygenates (8260B) and, <br /> • Conduct aquifer-pumping test on both EW wells. Collect and <br /> characterize purged ground water prior to proper disposal and <br /> • Conduct liquid and vapor extraction pump test for evaluation of <br /> `multi-phase testing' (MPT) as additional remedial activity. <br /> • Submit report of findings in the format of an updated Site Conceptual <br /> Model, to include an evaluation of remedial feasibility. <br /> COMMENTS AND CONDITIONS: <br /> • SJCEHD recommends that the deep extraction well's screened <br /> interval extend no further than 85' below grade to ensure adequate <br /> separation from the lower sand unit. <br /> • Soil analyses for 1,2-DCA, EDB and lead in the MW-5 area are not <br /> available. Selected soil samples collected from replacement borings <br /> in this area are to be analyzed for these constituents (8260B) or <br /> provided from existing CPT-4 analyses. <br /> • Methanol and Ethanol to be analyzed by EPA Method 8015m. <br /> • Submit a well installation ($89) and well destruction ($60) fee with <br /> each application by December 27, 2004. <br />
