The containers <br /> were: <br /> -one approximately 1.5 gallon RCRA hazardous waste container located in the block cage with medication including <br /> ibuprofen, vitamins, naproxen, and travel sickness medication was missing accumulation start date, generator name <br /> and address, composition, physical state, and hazardous properties on the label <br /> -one approximately 1.5 gallon RCRA hazardous waste container located in the block cage with an unknown liquid <br /> and medication was missing the accumulation start date, generator name and address, composition, physical state, <br /> and hazardous properties <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br /> action statement and supporting documentation to the San Joaquin County Environmental Health Department <br /> (EHD)within 30 days. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br /> complete report on October 21, 2019 which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Note: According to James "Jim" McCarthy, EH&S Manager, Lodi Memorial Hospital West located at 800 S <br /> Lower Sacramento Road provides Outpatient Physical Therapy, occupational health (physicals), employee <br /> health (physicals), and home health and does not generate hazardous waste.
