Monitoring records <br /> include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br /> records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site and submit copies to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br /> reports. The incorrect and missing information included the following from the inspection reports: <br /> -The D.O's reports dated 10/31/19, 07/10/19, 06/13/19, 5/14/19 and 4/16/19 left blank the owner/operator name <br /> (section IV) <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. <br /> OBSERVATION: A copy of the current UST operating permit was not found on site. <br /> REGULATION GUIDANCE: A copy of the current UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the current permit and retain it on site. <br /> This is a minor violation. <br /> FA0003787 PR0232437 SCO01 01/24/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
