SAN J O A R N Environmental Health Department -:a U ;� i _# �V AMENDED 6/19/2020 Underground Storage Tank Program Inspection Report Facility Name: Facility Address: Date: RYDER TRUCK RENTAL#1071 3633 DUCK CREEK DR, STOCKTON January 09, 2020 SUMMARY OF VIOLATIONS (CLASS I,CLASS II,or MINOR-Notice to Comply) Item# Remarks CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. 5)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -Several items were marked as"NA"on the inspection report and were not documented in Section III. -Sections VII and VIII were left blank. -Tank and line tightness test information was left blank. -The designated operator documented visual inspections of sumps, but left the question regarding sump sensor locations blank. -The designated operator documented visual inspections of UDCs, but left the question regarding UDC sensor locations blank. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class ll. 211 CCR 2715(c)(2) No employee present that were trained in the operation and maintenance of the UST system by a DO. OBSERVATION: The designated operator employee training for employees was not performed. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a repeat violation, Class ll. Overall Inspection Comments: The monitoring system certification was conducted on 1/9/2020. The testing was not witnessed in its entirety.
