City Of Stockton, Department Of Housing And Redevelopment -2-
• Copy of the fust letter from the local regulatory agency naming you a responsible party and directing
you to cleanup the contamination at the subject site.
PRIORITY CLASS "B"
• You have requested a Priority Class"B"classification. In order to qualify for Priority Class`B"you
must have less than $7 million total annual revenues for the latest fiscal year prior to the date your
claim application was submitted. You have indicated on page six(6)of your claim application that
the Total Annual Revenues is$4,433,576. However, that doesn't appear to support the figures listed
in the Financial Transaction Report submitted for the year 2004. Page six of your Financial
Transaction Report(copy enclosed) shows the total revenues to be$12,708,163)(which exceeds the
$7 million annual revenues required for a Priority Class`B"). However, you may apply for one of
the following Priority Classifications (revise pages 5 &6 of your claim application to reflect this
change):
PRIORITY CLASS"C"OPTION
• For a Priority Class "C"you must submit documentation verifying that claimant does not employ
more than 500 full time or part time employees. Submit documentation supporting the number of
employees (i.e. Department of Employment Development(DE6) for the last quarter).
PRIORITY CLASS "D"OPTION
• No further priority classification documents are required for Priority Class "D".
NOTE: Failure to respond to this request within thirty(30)calendar days from the date of this letter may
result in an ineligibility determination of your claim.
If you have any questions, please contact me at(916) 341-5761.
Sincerely,
QSCIA`M
Barbara Rempel
Claims Review Unit
Underground Storage Tank Cleanup Fund
cc: Mr.Brian Newman (w/o enclosures) "Ms. Margaret Lagorio(w/o enclosures)
RWQCB,Reg. 5 -Sacramento San Joaquin County EHD
11020 Sun Center Drive 304 E Weber Ave, 3rd Floor
Rancho Cordova, CA 95670 Stockton, CA 95202-2708
Audrey Winters (w/o enclosures)
Attorney at Law
11230 Gold Express Drive, 310-266
Gold River, CA 95670
California Environmenta/Protection Agency
ZA
O R"Yded P.P,
