CORRECTIVE ACTION STATEMENT This statement addresses the violations found at Micke Grove Park during a routine inspection. Item#20— 275 gallon container of used oil was not labeled properly. It is now labeled as"Hazardous Waste" with the generator name and address, contents, properties, hazardous characteristics and accumulation date. 55 gallon drum of asphalt handling material (asphalt emulsion)was disposed of on 10/9/08 (paperwork is included). Item#10— Solids from re-cycling wash rack not documented as non-hazardous. Analysis is being performed but is not yet complete. The results will be forwarded as soon as they are available. Item#15 —Eye wash equipment not near enough to 275 gallon tank of used oil. Eye wash station has been provided with in 8 feet of used oil tank. Item# 16—Eye wash station on Ford truck(license number 04958 1)not maintained. Eye was station on the truck in question has been serviced. Item#27, 30, 31, 33 —Disposal records, manifests, bills of lading for past three years were not available. All available records are being forwarded with this statement. Item#88—Used oil, used oil filters and asphalt handling material containers not marked properly with accumulation start dates. Asphalt handling material ( asphalt emulsion) has been disposed of, records are included with this statement. Used oil wand used oil filter containers are properly labeled and dated., Item# 90—Modified Emergency Coordinator information was not available. Emergency Coordinator information has been updated and posted. Item#91 —Five gallon containers of hydraulic fluid not marked as empty or the date emptied. Containers now properly marked or disposed of. Item# 94— SPCC Plan was not available for review. SPCC Plan is not necessary for this site because of the small volume of hazardous waste stored(used oil under 1320 gallons).
