Tank Closure, failed to remove/decon all waste and manage them as HW - <br />A 2000 gallon used oil above ground storage tank located in the south west corner of the property was <br />not closed properly. The tank came from A Plus recycling according to Mr. Hernandez. Remove/ <br />decontaminate all remaining waste inside the tank before disposing/selling it. Provide documentation <br />that all contents were managed as hazardous waste by October 1, 2010. <br />#60. Failed to keep signed copy for three years - <br />Upon reviewing the records on site, the following hazardous waste manifest signed off copies could not <br />be located: 000260515GBF - 2/6/07 and 00270988FLE - 9/24/09. Provide a copy of the signed off copy <br />by October 1, 2010. <br />#79. Failed to store universal waste lamps in a closed container and in a manner to prevent breakage <br />and release to the environment - <br />Several used lamps are being stored in a truck trailer located along the western fence line behind the <br />former paint shop. Provide a container for the used lamps and keep it closed to prevent breakage. <br />Provide documentation for providing a container by October 1, 2010. <br />#84. Failed to demonstrate the length of time the UW has been accumulated from the date it became a <br />waste or was received - <br />Upon reviewing the waste disposal receipts on site, records were not found for recycling used lamps. <br />Make arrangements to have the used lamps disposed of properly and provide documentation for the <br />disposal by October 1, 2010. <br />Submit the return to compliance certification with a corrective actions statement and supporting <br />documentation by October 1, 2010. <br />Note: This facility is going through the process of closure. Please inform the EHD when the facility <br />closes so that the hazardous waste permit can be inactivated. The following violation was corrected on <br />site: Tanks of waste coolant and used oil were labeled, but the writing had worn off for the accumulation <br />start dates. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115). <br />THIS FACILITY IS SUBJECT TO ECTIAT ANY TIME AT EHD'S CURRENT HO RLY RATE. <br />-REINS <br />EFfirlinspecto <br />Recei By: <br />EHD 23-02-003 Rev 06/01/10 CONTINUATION FORM <br />
