Contractor shall comply with all provisions of Executive Orders <br />11625 and 12138, which state that it is the policy of the <br />Government that minority and female business enterprises shall <br />have the maxium practicable opportunity to participate in the <br />performance of this contract. The Contractor agrees to use its <br />best efforts to carry out this policy in the award of <br />subcontracting to the fullest extent consistent with the <br />efficient performance of this Contract. The Contractor also <br />agrees to comply with PCC 10115.2 and agrees to allow awarding <br />department or its delegate the right to review, obtain, and copy <br />all records pertaining to the performance of this contract; <br />relevant information requested by awarding department; and access <br />to its premises during normal business hours with reasonable <br />notice. Contractor agrees to maintain such records for a period <br />of three years after final payment of the contract. <br />Insurance Requirements <br />Contractor hereby warrants that it carries Worker's compensation <br />Insurance for all its employees who will be engaged in the <br />performance of this. contract and agrees to furnish to State <br />satisfactory evidence thereof at any time the state may request <br />the same. The Contractor further warrants that it carries other <br />applicable insurance such as Professional Liability, General <br />liability, and Business Auto and will provide evidence if so <br />requested. <br />Law Governing <br />It is understood and agreed that this contract shall be governed <br />by the laws of the State of California as to both interpretation <br />and performance. <br />Evaluation of Contractor <br />Performance of the Contractor under this Agreement will be <br />evaluated when contract exceeds $5,000.00. The evaluation shall <br />be prepared on Contractor Evaluation Form, STD 4. A copy of the <br />evaluation will be sent to the Legal Office, Department of <br />general Services. The Legal Office will act as a central <br />depository for all State agencies making evaluation or desiring <br />information on a contractor's record with the State and will sen <br />a copy of any evaluation report to any State agency upon request. <br />Invoicing <br />Upon completion of the tasks or at expiration of the contract, <br />the Contractor shall submit, in quadruplicate, and invoice <br />containing contract number, name, tasks completed, number of <br />hours required, and any travel expenses (payable at State Rate <br />pursuant to Code Sections 599.615-636) with supporting vouchers. <br />
