These forms are being submitted to provide updated information <br /> on each of our stores, pursuant to the requirements of Section 25201.5(4)(7) of the California Health and Safety <br /> Code. <br /> The procedure for managing the small amounts polycarbonate coating waste generated at LensCrafters' <br /> stores is to collect the material directly from a coating machine, transfer it into a small tray, and cure it under Uv <br /> light until it is hardened.
