SUMMARY <br /> May 6, 2003 <br /> Mr. Leo Valdez of the Department of Toxic Substances Control (DTSC)conducted a <br /> compliance inspection with Ms. Kasey Foley, Lead Senior,RENS, and Mr. John Jackson, <br /> EHS (Attachments 1&2). The objective was to verify compliance with hazardous waste <br /> and tiered permitting statutes and regulations. Mr. Jackson asked to conduct an inspection <br /> and was told that Doug Baker, the manager with authority to permit an inspection, was <br /> out on medical leave. The current manager Mike Tucker offered to call Mr. Doug Baker <br /> and obtain his permission to conduct an inspection. Doug Baker spoke with Mike Tucker <br /> over the phone and authorized Mike Tucker to show the facility,but advised Mr. Tucker <br /> to recommend that Mr. Jackson come back another time to view documents. Mr. <br /> Jackson,Mr. Valdez, & Ms. Foley began the inspection by viewing the facility, asking <br /> questions, taking pictures, and taking samples(taken by Leo Valdez of the DTSC). The <br /> samples consisted of buffer wheel floor sweep,polishing wheel floor sweep, and metal <br /> salts from an extremely large open hazardous waste tank. A duplicate set of hazardous <br /> waste samples was left on site and a chain of custody was started. <br /> May 13, 2003 <br /> Mr. Valdez and Mr. Jackson went back to the facility to conduct the paper portion of the <br /> tiered permitting, and hazardous waste inspections. The following were unavailable: <br /> Financial assurance data, Phase I Environmental Assessment (PEA)report, and an <br /> approval letter from a state certified engineer for tanks at the facility. <br /> Doug Baker was instructed to find these documents and to fax them to Mr. Jackson. <br /> During this inspection Mr. Jackson left a copy of photographs taken on 5/5/03. <br /> May 15,2003 <br /> Mr. Jackson discussed the details of the inspection with Supervising REHS Doug Wilson, <br /> and decided that this site needed to have a OSHA referral for confined space issues and <br />
