issues surrounding cyanide tanks in close proximity to acid tanks. A phone referral was <br /> made to OSHA. <br /> May 20,2003 <br /> Mr. Jackson went to the facility to give Doug Baker completed copies of the tiered <br /> permitting and hazardous waste inspection reports. Mr. Jackson explained the violations <br /> and took time to go through each photograph. At this time Mr. Jackson also expressed <br /> the time limitations for specific violations. At the end of the meeting Doug Baker was <br /> encouraged to ask questions. <br /> June 9, 2003 <br /> Mr. Jackson placed a call to Violetta Mislang of the DTSC and inquired about the <br /> existence of a PEA Report. She agreed to look into the matter and asked Mr. Jackson to <br /> fax a copy of the notification form along with the existing tiered permitting authorization <br /> document. <br /> June 11,2003 <br /> Mr. Jackson found and faxed a copy of a notification form(Attachment 3) from 1995. <br /> Mr. Jackson was unable to find any tiered permitting authorization documentation. Mr. <br /> Jackson made a request of Ms. Mislang to forward all needed PEA documents to Delta <br /> Plating. <br /> June 16,2003 <br /> Mr. Jackson received a call from Doug Baker. He had questions about compliance <br /> issues. Mr. Jackson informed him that there appeared to be no records of a PEA ever <br /> being done. Mr. Jackson also informed Mr. Baker that the facility was not currently . <br /> authorized to treat cyanide. Mr. Jackson explained that in order to treat cyanide the <br /> facility would need to seek approval from the DTSC. Mr. Jackson advised him to contact <br /> Mr. Valdez and handle this issue in an appropriate manor. Doug Baker expressed a <br /> willingness to comply with all issues,but expressed concern over meeting the time frame <br /> for the disposal of hazardous waste. He asked if Mr. Jackson could grant him a time <br /> extension. Mr. Jackson expressed the importance of timely disposal and informed Mr. <br /> Baker that he was unsure of the authority to grant a time extension. Mr. Jackson did <br /> however acknowledge Mr. Bakers request and asked that Mr. Baker send pictures of the <br /> corrected violations upon correction. Mr. Baker estimated that the hazardous waste <br /> would be removed by July 11, 2003. Mr. Jackson faxed a packet to Delta Plating <br /> containing cyanide order documentation(Attachment 4). In the packet Mr. Jackson <br /> documented a request for the PEA information, and copies of tiered permitting <br /> authorization. <br /> June 18, 2003 <br /> Mr. Jackson faxed a copy of a blank financial assurance document to Delta Plating <br /> (Attachment 5). <br /> July 31,2003 <br /> Mr. Jackson Placed a call to Asha Aurora of the DTSC to check if Delta plating had filed <br /> an order to treat cyanide, she said that they had recently applied for the order but that she <br /> had not received any of the requested sample documentation needed in order to assess the <br /> site. Mr. Jackson filed the returned DTSC sample results from the May 6,2003 <br />
