The information on this monitoring <br /> program are conditions of the operating permit. The permit holder must notify San Joaquin County <br /> Environmental Health Division,phone (209) 468-3420 within 30 days of any changes to the monitoring <br /> procedures,unless required to obtain approval before maldng the change. <br /> Required by Sections 2632(d) and 2641(h) CCR. <br /> Facility Name Dameron Hospital <br /> Facility Address 525 W. Acacia Street, Stockton, CA 95203 <br /> A. Describe the frequency of performing the monitoring: <br /> Tank continous monitoring by 'Watch- Engineer cin- �.��rX. 2 haurs a day <br /> Piping continous monitoring by Watch Frtginaar nn r., 24 hours a day__ <br /> B. What methods and equipment, identified by name and model, will be used for <br /> performing the monitoring: <br /> Tank Leak Alert Model LA-02 Separate Vapor and Liquid Detection Monitor <br /> PipingyLeak Alert Model LA-02 Separate Vapor and Liquid Detection Monitor <br /> C. Describe the location(s) where the monitoring will be performed (facility plot plan <br /> should be attached): <br /> Monitoring is performed in the boiler room see attached facility <br /> plot p an <br /> D. List the name(s) and ride(s) of the people responsible for performing the monitoring <br /> and/or maintaining the equipment: <br /> John Dakopolos, Chief Engineer" <br /> E. Describe the preventive maintenance schedule for the monitoring equipment. Note: <br /> Maintenance must be in accordance with the manufacturers' maintenance schedule <br /> but not less than eve17 12 months: Monitoring equipment checked annually <br /> by Champion Industrial Contractors <br /> G. Describe the training necessary for the operation of UST system, including piping, <br /> and the monitoring equipment: Watch Engineer trained to turn off pump <br /> in case of alarm and call Chief Engineer. <br />
