The designated operator signed the January and February 2016 reports during the inspection. <br /> The designated operator inadvertently listed the date for the secondary containment testing as January 28,2015 <br /> instead of January 27,2015 on all of the designated operator monthly inspection reports from July 2015 through July <br /> 2016. The designated operator corrected the secondary containment testing dates on all of the designated monthly <br /> inspection report during the inspection. <br /> These were corrected on site. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2636(f)(2)Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87-product mechanical line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3.0-gallon per hour leak at ten pounds per square inch (psi)of pressure. The service technician <br /> adjusted the VMI 99-LD2000 leak detector twice before the test passed. <br /> This was corrected on site. <br /> This is a repeat violation, Class Il. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility environmental manager on the day of inspection. The <br /> Environmental Health Department(EHD) has written the complete report which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS):UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br /> applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written <br /> Agreement, if applicable, can be stored at the facility or uploaded into CERS. <br /> Provided: Schedule of free CUPA classes. <br /> Page 4 of 6 <br />
