The designated operator failed to sign the designated operator monthly inspection reports for January 5, 2016 and February 9, 2016. The designated operator signed the January and February 2016 reports during the inspection. The designated operator inadvertently listed the date for the secondary containment testing as January 28,2015 instead of January 27,2015 on all of the designated operator monthly inspection reports from July 2015 through July 2016. The designated operator corrected the secondary containment testing dates on all of the designated monthly inspection report during the inspection. These were corrected on site. This is a repeat violation, Class II. 318 CCR 2636(f)(2)Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87-product mechanical line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3.0-gallon per hour leak at ten pounds per square inch (psi)of pressure. The service technician adjusted the VMI 99-LD2000 leak detector twice before the test passed. This was corrected on site. This is a repeat violation, Class Il. Overall Inspection Comments: An inspection checklist was provided to the facility environmental manager on the day of inspection. The Environmental Health Department(EHD) has written the complete report which replaces the initial checklist. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the complete inspection report. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS):UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if applicable), and the Owner Statement of Designated Operator Compliance.
