Financial responsibility documents for 2013 have not been submitted to the EHD. The most recent <br />financial responsibility records on file are dated March 28, 2012. Current financial responsibility <br />documents are required to be submitted annually. Complete and submit a copy of the financial <br />responsibility by May 1, 2013. <br />REPEAT VIOLATION <br />108. Secondary containment testing was last performed on November 19, 2009 and was due by <br />November 2012. Testing has not been completed and is five months past due. Secondary containment <br />testing is required once every 36 months. Immediately schedule this test and provide 48 hours <br />notification to the EHD. <br />REPEAT VIOLATION <br />109. Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on December 21, 2011 and a test report has not been submitted to the EHD. A copy of the <br />test report must be submitted within 30 days of the tests. Immediately submit a copy of the test report to <br />the EHD. <br />109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on December 21, 2011 and was completed on April 1, 2013, four months past due. <br />These tests are required once every 12 months.
