Financial responsibility documents for 2013 have not been submitted to the EHD. The most recent financial responsibility records on file are dated March 28, 2012. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility by May 1, 2013. REPEAT VIOLATION 108. Secondary containment testing was last performed on November 19, 2009 and was due by November 2012. Testing has not been completed and is five months past due. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. REPEAT VIOLATION 109. Annual monitoring system certification, leak detector testing, and spill container testing was performed on December 21, 2011 and a test report has not been submitted to the EHD.
