Insertion Report <br />1 _n <br />Facility Name: <br />Facility Address: <br />Date: <br />SHELL FOOD MART <br />2320 N EL DORADO ST STOCKTON <br />February 13 2018 <br />SUMMARY OF VIOLATIONS <br />(CLASS 1, CLASS 11, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the designated operator <br />monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: <br />Setup Data Warnings on 10/31/17 and 7/31/17 on the L6 diesel annular. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was documented and <br />responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />301 <br />HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br />The fuel filters in all four dispensers were not contained in the UDC. The underground storage tank system shall be <br />operated to prevent unauthorized releases. Immediately take all necessary steps to ensure any leaks from fuel filters <br />will drain into the UDC, under permit by the EHD if necessary. <br />This is a Class II violation. <br />313 <br />HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product -tight. <br />A fuel filter in dispenser 2/4 was observed dripping fuel. All primary containment for the UST system must be tight. <br />Immediately stop all primary containment leaks, under permit if necessary. <br />This is a minor violation. <br />314 <br />HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br />The fuel filters under all dispensers are not contained within the UDC and do not currently have secondary <br />containment. Secondary containment must be provided for the UST system. Immediately have a properly licensed, <br />trained, and certified contractor repair or install adequate, approved secondary containment for the fuel filters under <br />permit and inspection of the EHD. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by March 15, 2018. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br />applicable), and the Owner Statement of Designated Operator Compliance.
