A shear <br />valve repair was conducted on 2-9-2017 in conjunction with a dispenser replacement and the EHD was not notified <br />of this repair, as required. These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. Immediately submit the following information to the EHD concerning this <br />repair: 1) Scope of work, 2) Date of repair, 3) Proof of functionality verification. A copy of the pertinent maintenance <br />record was received at the time of inspection. All shear valves were tested with passing results today. <br />This was corrected on site. <br />This is a repeat violation, Class 11. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training log was lacking the hire dates for employees that were trained and hired <br />after 2005. The designated operator shall train facility employees for which he or she is responsible in the proper <br />operation and maintenance of the UST system once every 12 months and maintain the log of this training. This was <br />corrected on site. <br />This is a Class 11 violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 3-24-2017. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following LIST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br />applicable), and the Owner Statement of Designated Operator Compliance.
