EHD must be <br />present to witness any subsequent inertion of the tanks. <br />The underground storage tank shall be inspected by the owner or operator at least once every three <br />months to verify that the temporary closure measures are still in place. The inspection shall include but <br />is not limited to: <br />1) Visual inspection of all locked caps and concrete plugs <br />2) If locking caps are used, at least one shall be removed to determine if any liquids or other <br />substances have been added to the underground storage tank or if there has been a <br />change in the quantity or type of contents added when initially inerting the tanks. <br />EHD must be present to witness all quarterly inspections. Please be aware that all EHD staff time <br />associated with these inspections will be billed at the current hourly rate. Submit copies of quarterly <br />inspection reports to the EHD within 30 days of each inspection. <br />Power service shall remain disconnected from all pumps associated with the use of the underground <br />storage tank. <br />The underground storage tank system must continue to comply with all repair and recordkeeping <br />requirements, release reporting and investigation requirements, and release response and corrective <br />action requirements. The underground storage tank may be in temporary closure for a maximum of <br />twelve consecutive months. <br />ALL END STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Title: <br />EHD 234 Rev (/01/11 / Page 3 UST INSPECTION REPORT <br />
