The following is an itemized list of underground storage tank violations that have not been addressed for EL DORADO FOOD MART as of May31, 2017. Open violations from August 09, 2016 inspection Violation #104 - UST operating permit application for facility and tank information not submitted or current. The Board of Equalization account number TK STF 44045016 is closed and not valid. Immediately contain the Board of Equalization and validate this account. Violation #105 - Failed to have an approved UST monitoring plan. For the underground storage tank TA0515512 monitoring plan, beneath Pipe Monitoring is Performed Using the Following Methods, change mechanical line leak detector MLLD Manufacturer to VMI and change the MLLD model to 99LD2000. For both the 87- and 91 -product underground storage tanks, beneath Recordkeeping, change "no" to "Yes" for Visual Inspection Records since this facility is keeping daily inspection records. Violation #121 - Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on August 12, 2015 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by September 11, 2015. This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Outstanding violations from the 2015 monitoring system certification include: The Environmental Health Department did not receive the 2014 Return to Compliance certification for outstanding violations; and an approved copy of the monitoring plans were not available for inspection. The second violation will be removed since a monitoring plan was available today during the inspection. Violation #209 - Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator listed the date of 9/22/15 for "Spill container structure (bucket) testing was completed within the past year" on the October 26, 2015 through July 27, 2016 designated operator monthly inspection reports. The date that the 91 -product spill container (that initially failed during the annual testing date of 8/12/15) passed was 8/31/2015. The designated operator listed " NA" for "Spill container structure (bucket) testing was completed within the past year" on the August 28, 2015 and September 28, 2015 designated operator monthly inspection reports. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done.
