BOX 31465 i� <br />STOCKTON, CA 95213 <br />Re: ELDORADO FOOD MART SR0076414 <br />1901 S ELDORADO ST <br />STOCKTON, CA 95206 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to install new Gilbarco Dispensers and new Bravo conversion frames <br />for the two existing dispensers and tighten existing hose clamps on electrical penetrations in <br />UDC % on the underground storage tank (UST) system at the above noted address. <br />The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). According to <br />the service request, no piping modifications will take place during the proposed <br />conversion frames installation. <br />a. Verification of conversion frames installation per the manufacturer's installation <br />instructions. The Bravo Conversion Frame Evaluation Forms shall be available <br />for review at the time of inspection. <br />b. Verification of the dispenser footprints contained by the under dispenser <br />containment sumps (UDCs) and verification that all potential product piping <br />leaks will be captured in the UDC. <br />c. Visual inspection of the piping within the dispensers for any product leaks <br />d. Witness secondary containment testing of UDC 3/4 after returning the system to <br />the operating condition. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4.
