The missing alarms include: <br /> DO monthly inspection report for 7/25/16 is missing the L4, L10 and L11 alarm histories on the panel printout. <br /> DO monthly inspection report for 8/5/16 is missing the L3 and L11 alarm histories on the panel printout. <br /> DO monthly inspection report for 9/30/16 did not document the L1:Disp 1-2 sensor out alarm on 9/13/16; L2: Disp <br /> 3-4 fuel alarm on 9/13/16; L3: Disp 5-6 sensor out alarm on 9/12/16 and 9/13/16; L4: Disp 7-8 sensor out alarm on <br /> 9/12/16 and L11:87-3 STP sensor out alarm on 8/17/16. <br /> DO monthly inspection report for 10/7/16 is missing the L7 and L11 alarm histories on the panel printout. <br /> DO monthly inspection report for 11/18/16 did not document the high water alarms and high water warnings for <br /> T1:Unleaded West 87 on 10/10/16 and 10/13/16; T2:Unleaded Center 87 on 11/7/16, 11/11/16, and 11/16/16; <br /> T3:Unleaded East 87 on 10/29/16, 11/7/16, and 11/16/16; and T4:UL Premium 91 on 10/25/16, 10/19/16, and <br /> 11/11/16; and did not document the L1:Disp 1-2 fuel alarm on 10/16/16. <br /> DO monthly inspection report for 12/16/16 did not document the high water alarms and high water warnings for <br /> T2:Unleaded Center 87 on 12/4/16, 12/7/16, and 12/11/16; T3:Unleaded East 87 on 12/2/16, 12/4/16 and 12/11/16; <br /> T4:UL Premium 91 on 11/20/16 and 12/7/16. <br /> DO monthly inspection report for 1/11/17 is missing the L5 alarm history on the panel printout. <br /> DO monthly inspection report for 2/27/17 did not document the high water alarms and high water warnings for <br /> T2:Unleaded Center 87 on 2/17/17, 2/21/17 and 2/24/17; T3:Unleaded East 87 on 2/12/17, 2/17/17, and 2/24/17; <br /> and T4:UL Premium 91 on 1/28/17, 2/12/17 and 2/21/17. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 315 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87-Master fill sump and under dispenser containment (UDC) 5-6. If water could enter <br /> into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The <br /> service technician removed approximately four cups of liquid from the 87-Master fill sump and approximately one <br /> cup of liquid from UDC 5-6. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 27,2017. <br /> Note: The 91-product spill container did not contain a capacity of five gallons. When the 91-product spill <br /> container fails, the 91-product spill container shall be replaced with a spill container that has a minimum capacity <br /> of five gallons, is protected from galvanic corrosion, and has either a drain valve which allows drainage of the <br /> collected spill into the primary container or provides a means to keep the spill container empty pursuant to CCR, <br /> FA0003632 PR0231094 SCO01 03/28/2017 <br /> EHD 23-01 Rev.09/22/16 Page 5 of 6 Underground Storage Tank Program OIR <br />
