The following is a Class 11 Repeat violation: <br />310, and the following is a Minor -Notice to Comply: 105 <br />105. UST tank forms on file with the EHD listed UDC to be fiberglass when it is actually steel; therefore <br />the forms are not current. Any change of information on these forms must be submitted to the EHD <br />within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to <br />the EHD. The forms were updated on site by the manager; the violation was corrected on site. <br />310. (Repeat) About 2 gallons of liquid was observed in the 87 -octane fill sump, and about 2 cups of <br />liquid in the 91 -octane fill sump. If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a <br />hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br />The liquid was removed by technicians and disposed to hazardous waste container on site. Submit <br />document to EHD how you will ensure that the sumps is maintained free of liquid in the future by <br />September 24, 2012. <br />NOTES (following are not violations) <br />The September 2011 and October 2011 designated operator inspection report listed the wrong ICC <br />certification expiration dates of the designated operators. The designated operators ICC certifications <br />were valid during these two months. <br />Technician switched the battery on the monitoring panel after the alarm history was printed but before <br />the monitoring system certification was conducted. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUI�JECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />lnl&,,.
