The 91 turbine relay did not turn off the turbine when the turbine sump sensor was tested today. <br />Therefore, positive shutdown does not work as specified in the facility monitoring plan. Correct <br />immediately by having a properly licensed, trained, and certified contractor replace the failed relay with a <br />functional component and obtain a permit within one business day from the Environmental Health <br />Department (EHD). <br />Notes: <br />1. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by August 7, 2011. <br />2. A line test was done today.
