<br />The San Joaquin County Environmental Health Department (EHD) has completed the review of
<br />the application for a permit to conduct an EVR phase 11 upgrade with a Veeder Root carbon
<br />canister. ISD will not be installed. The permit has been approved with the following conditions:
<br />Conditions of approval:
<br />1. Contact the local fire department, local planning or building department and the San
<br />Joaquin Valley Air Pollution Control District for any applicable permits.
<br />2. Any changes shall be submitted to the San Joaquin County Environmental Health
<br />Department (EHD) for approval prior to commencing work. All new addenda supercede
<br />the previous ones.
<br />3. Contact Garrett Backus (209-468-2986) to arrange the following inspection(s) (a 48-hour
<br />notice is required):
<br />a. Inspection of the monitoring panel to verify that the monitoring system alarm
<br />history is still intact after completion of the EVR upgrade.
<br />4. A "Certification of Installation / Modification" form shall be submitted by the owner or
<br />the owners' agent [pursuant to (CCR) 2635(d)] before this upgrade will be considered
<br />complete by EHD.
<br />5. All components must be certified by an independent testing organization and labeled as
<br />required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of
<br />Regulations (CCR), Title 23 Chapter 16 Table B for complete list). The component must
<br />bear the appropriate mark as required by the independent testing organization.
<br />6. Qualified service technicians must possess or work under the direct and personal
<br />supervision of an individual physically present at the work site who possesses all of the
<br />following:
<br />a. One of several permissible valid, current contractor's licenses issued by the
<br />Contractors State License Board (may be issued to the installation company), and
<br />b. All valid, current California UST certificates issued by the International Code
<br />Council (ICC), that are applicable to the work being performed, and
<br />c. A valid, current certificate of training from the manufacturer(s) of the
<br />component(s) being tested, repaired, or serviced;
