All monitoring equipment shall be maintained to
activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak.
Corrected during the inspection by the testing technician by adjusting the Bravo chain.
21) Approximately one inch of liquid was observed in the 91 tank annular space. The liquid appeared to
be water. The annular liquid sensor was at the lowest point in the tank riser and did not go into alarm.
The sensor also passed the annual test done today.
If water could enter into the secondary containment by precipitation or infiltration, it must be removed
and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per
Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the 91 tank annular space
is maintained free of liquid.
23c) The secondary containment test date (August 27, 2009) on the June 2010 designated operator
report is incorrect. The correct date is June 8, 2008. Use the correct date on future designated operator
reports. Same violation is on the July 6, 2010 inspection report.
Notes
1) Spill containers were tested July 6, 2010 at the tank owner's request. The remainder of the annual
testing was done today.
2) This inspection will be billed to the facility at the Environmental Health Department hourly rate since
this is second inspection done this fiscal year. Inspector was on site 9 am - 12 noon.
ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115).
THIS FACILITY IS UBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
EHD Inspe
Received By:
Title:
`i -W( v
EHD 23-02-0 Rev 06/01/10 CONTINUATION FORM
