Contact the local fire department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. If any new equipment needs to be installed this office must be given cut-sheets prior to <br /> installation for approval. <br /> 3. If concrete is broken near any underground storage tank a third party certified tank integrity <br /> test must be performed and results must be submitted to this office prior to the station <br /> returning to service. <br /> 4. EHD must witness a 30-minute pressure test on all repaired/replaced piping prior to backfill. <br /> 5. All piping must slope to a containment sump with an audible and visual alarm. <br /> 6. All metal equipment in contact with backfill must have corrosion protection. <br /> 7. Contact Dina Abate at (209) 468-3468 to arrange an appointment to witness the excavation <br /> of the piping. <br /> 8. Allow 48-hour notice for all appointments. <br /> 9. Soil sampling may be necessary per Environmental Health assessment. <br /> 10. Permit expires April 14, 2003. Note: If an extension is needed a written request must be <br /> submitted to this office prior to the expiration date. <br /> 11.
