The information that is <br />not current: <br />-TA0508150- In the section for Product/ Waste Piping Construction- Primary Containment, Secondary Containment <br />and Piping/Turbine Containment Sump information is not listed. In the section Vent, Vapor Recovery (VR) and Riser/ <br />Fill Pipe Piping Construction- Vent Piping Transition Sumps information is incorrect. <br />-TA0508152- In the section for Product/ Waste Piping Construction- Piping/Turbine Containment Sump information is <br />not listed <br />Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br />update the required information, and submit for review by the EHD. <br />This is a Class II violation. <br />107 <br />CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />An accurate UST Monitoring Site Plan was not submitted. The site plan did not indicate the monitoring of the UDCs <br />and what type of sensors monitor the UST system. A site plan must be submitted identifying the locations where <br />monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />This is a minor violation. <br />115 <br />CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. <br />A new designated operator was hired on March 14, 2016 and notification was not provided to the EHD within 30 days <br />of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the <br />designated operators for this facility. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by June 17, 2016 . <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br />applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written <br />Agreement, if applicable, can be stored at the facility or uploaded into CERS. <br />Page 4 of 5 <br />
