What are you going to do about notifying our agency if a new designated operator, working for Bellshire, is hired? Are you going to notify our agency within 30 days? Please resubmit your responses and address the 115 violation from 2016. Also, you have not addressed item 121 from the 2017 inspection - Failed to submit a written response within 30 days of receiving an inspection report. You need to address this violation also.
