California <br />94-261-94-20 <br />At the request of Quik Stop Markets, Inc. (Quik Stop), Smith Technology <br />Corporation (Smith Technology) is issuing this addendum to the underground <br />storage tank (UST) removal report referenced above (Smith Technology, October <br />25, 1996). This addendum includes the uniform hazardous waste manifests and <br />tank receipt and destruction certificates from the transport and disposal of the <br />UST's removed from this site. These documents were not available at the time <br />the above referenced report was issued. <br />As discussed in the above referenced report, on September 18, 1996 four UST's <br />were removed from the site consisting of two 8,000 gallon UST's and two 10,000 <br />gallon UST's. These UST's were transported from the site to the Erickson, Inc. <br />facility in Richmond, California under uniform hazardous waste manifests numbers <br />96430846, 96430851, 96430832, and 96430847. Copies of these manifests are <br />included as Appendix A. <br />After delivery to the Erickson, Inc. facility the UST's were cut open, processed <br />and destroyed by Certified Services Company.
