SEPTEMBER 24, 1996 <br /> TO: UST FILE 9321 THORNTON ROAD <br /> FROM: JERRY YOSHIOKA, RENS <br /> SUBJECT: UST INSTALLATION <br /> On September 23, 1996, 1 was asked by Michael Kith, SR. RENS, to <br /> witness the setting of two underground storage tanks at 9321 <br /> Thornton Road. I inquired on the manufacturer of the tank and <br /> was told that the two tanks were XERXES tanks with brine-filled <br /> secondary containment. I requested and received the installation <br /> plans and the manufacturer's installation information. I <br /> inquired about the pre-installation testing and was told that a <br /> pressure test of 5 psig for 30 minutes of the primary tank was in <br /> order. <br /> On September 24, 1996, 1 arrived at the site at 0945 hours and <br /> met Ron Williams of Wilkey Engineering. We waited for 15 minutes <br /> for the crane to arrive. I asked Mr. Williams about the pre- <br /> installation pressure testing of the primary tank and was told <br /> that "Mike Kith waived the primary test due to the brine-filled <br /> secondary", therefore he was unprepared for the test. I informed <br /> him of my discussion with Mike K. on the previous day regarding <br /> the intent for a pre-installation test. I was unable to locate <br /> any reference to the waiving of the primary tank in the file, <br /> however Mr. Williams was adament that Mike had agreed about the <br /> waiver. I informed Mr. Williams that since he did not bring the <br /> compressor and fittings for the pressure testing based on his <br /> understanding with Mike, he could set the tanks after the outer <br /> tanks had been inspected. The primary tanks would then be <br /> pressure tested in the excavation the following day. <br /> I informed Mr. Williams that if Mike Kith had not agreed to the <br /> waiving of the testing then he may be required to remove the <br /> tanks from the excavation and/or that the installation permit may <br /> be revoked. He said that "that won't happen because I will take <br /> it to court because Mike agreed to waiving the primary test. " <br /> As of 1120 hours, Wilkey Engineering was to make an appointment <br /> with the office for an inspection as early as possible for <br /> September 25, 1996, for the primary pressure testing. <br /> ct, Froc) <br />
