The Bravo box float and chains in the dispenser 1/2 and 3/4 sumps failed to stop the flow of product at <br /> the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> cleaned the float cups of debris and verified functionality. <br /> 18. The 87 octane spill container failed when tested. All spill containers shall have a minimum capacity <br /> of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. The spill <br /> container was replaced and retested during the inspection. Obtain a permit from the EHD for this work <br /> within one business day. If a permit is not obtained within one business day, a "Failure to Obtain a <br /> Permit' penalty will apply. <br /> 21. Liquid was observed in the 87 STP, 87 fill and 91 fill sumps. If water enters the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. The liquid was <br /> removed by the technician and placed in the site hazardous waste drum. Ensure that the tank top <br /> sumps are maintained free of liquid. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspe r: Received By: Title: <br /> E 2 2-003 Rev 08/10/10 CONTINUATION FORM <br />
