Return to Compliance Certification.
2. A review of the July'09 repair dispatch logs or work orders did not reveal
any reference to L2,L3,or L4 sensor out alarms on 7/28/09,nor was there
any reference to L3 or L4 sensor out Alarms on 8/19/09. The work
order/invoice for the L3 alarm on 9/14/09,and the work order for the L3
alarm on 1/8/10 are attached. Walton Engineering(the D.O.)has been
cautioned about noting all alarms on the monthly inspection records, even
those suspected of being spurious probe signals.
3. The 5/6/10 D.O. Report showing 7/17/09 as the SB-989 test date is
partially correct. That was the date that secondary containment testing(91
STP sump only)was completed. All other system components passed on
6/23/09. According to my records, the actual due date for SB-989 testing
was August 2010.
4. The drums of hazardous waste on-site at the time of inspection were
picked up for disposal on 6/23/10. This includes 87 and 91 fill sump
water removed at the time of inspection.
