The Bravo box floats and chains in the UDC 1/2 sump (91) and UDC 3/4 sump (87 & 91) failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the floats and chains and verified functionality. REPEAT VIOLATION 310. Liquid was observed in the 87 and 91 fill sumps. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the fill sumps are maintained free of liquid. REPEAT VIOLATION Notice to Comply - Minor violations: (104, 107, 202) 104. Financial responsibility documents have not been submitted to the EHD. The most recent financial responsibility documents on file are dated May 1, 2013 through May 1, 2014. Current financial responsibility documents are required to be submitted annually. Immediately submit a copy of the financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required information, and submit for review by the EHD. Complete by June 7, 2014. 107. The plot plan / site map submitted in CERS does not identify the location of the monitoring panel. The monitoring program shall include a plot pian identifying the location(s) where monitoring wiii take place.
