For the regular and premium tanks under the <br /> section labeled"Vent,Vapor Recovery(VR)and Riser/Fill pipe Piping Construction," the riser pipe secondary <br /> containment currently indicates"none."The riser pipe is located within a sump, and should be changed to indicate <br /> fiberglass. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). <br /> Immediately log into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted.The current site plan on CERS is missing the general <br /> layout of underground piping. A site plan must be submitted identifying the locations where monitoring will be <br /> performed. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site. Last year's monitoring system <br /> certification test results were not found onsite. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and <br /> maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the <br /> field;(5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; <br /> (6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and <br /> monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Veeder root 208 sensor in the 91 STP sump initially failed when tested.the technician inverted the sensor, and <br /> found the float to be stuck.The technician retested the sensor hydrostatically, and verified functionality.The Veeder <br /> root 208 sensor in the 87 STP sump was found with damaged wiring. The sensor failed to activate an audible and <br /> visual alarm when tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be <br /> maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a <br /> leak. The sensor was replaced and retested during the inspection. A retrofit verification form was provided during <br /> inspection.This was corrected onsite. <br /> The Bravo box float and chain in UDC sump 1/2 failed to stop the flow of product at the dispenser when tested. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak.
