No other employees have been trained by the facility designated operator within the past twelve <br />months. The designated operator shall train facility employees for which he or she is responsible in the proper <br />operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />During the inspection, the facility designated operator completed the required training with Marcos Ortega III and <br />Natalie Biu and documented it. Continue to ensure that employees have been trained by the designated operator, and <br />maintain the list on site. This is a Class II violation. <br />325 <br />CCR 2635(b) Spill container failed to contain a minimum capacity of five gallons. <br />The North and South diesel spill containers failed when tested. All spill containers shall have a minimum capacity of <br />five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. The North spill container <br />cap was changed out. The South spill container riser components had dope applied and were tightened. Both spill <br />containers were retested during the inspection and passed. This is a repeat violation, Class Il. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 12-6-14. <br />Consent to perform the inspection, take photos and collect samples was given by Kelli Kishida, Secretary. This <br />facility's designated operator is James Williams (ICC expiration date: 6-4-15).
