The designated operator employee training for Marcos Ortega III and Natalie Biu was last performed on 10-15-13 and is no longer current. No other employees have been trained by the facility designated operator within the past twelve months. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms During the inspection, the facility designated operator completed the required training with Marcos Ortega III and Natalie Biu and documented it. Continue to ensure that employees have been trained by the designated operator, and maintain the list on site. This is a Class II violation. 325 CCR 2635(b) Spill container failed to contain a minimum capacity of five gallons. The North and South diesel spill containers failed when tested. All spill containers shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. The North spill container cap was changed out. The South spill container riser components had dope applied and were tightened. Both spill containers were retested during the inspection and passed. This is a repeat violation, Class Il. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 12-6-14. Consent to perform the inspection, take photos and collect samples was given by Kelli Kishida, Secretary. This facility's designated operator is James Williams (ICC expiration date: 6-4-15). The service technician was Rhome Desbiens (ICC expiration date -1-24-15), who had the following manufacturer's certifications: VMI exp. 2-14-15, OMNTEC exp. 11-12-15, SWRCB exp. 11-30-16, Petrotite exp. 4-29-16. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance.
