0 0 <br /> Occupational Safety and Health Association(OSHA) <br /> U.S. Department of Labor <br /> 200 Constitution Avenue, N.W.,Washington, DC 20210 <br /> Permit-Required Confined Space Entry for General Industry: Final Rule, 1993 <br /> Contractor/Applicator shall be familiar and comply with the rules and regulations of the Occupational <br /> Safety and Health Association (OSHA) as published in the Code of Federal Regulations, Part 1910, <br /> particularly: <br /> Subpart H Hazardous Materials <br /> 106, Flammable and Combustible Liquids <br /> Subpart I Personnel Protective Equipment <br /> 132, General Requirements <br /> 134, Respiratory Protection <br /> Subpart J General Environment Controls <br /> 145, Specifications for Accident Prevention Signs and Tags <br /> Subpart L Fire Protection <br /> 157, Portable Fire Extinguishers <br /> 1.3.2 Be certain that all procedures and equipment necessary for compliance with applicable safety <br /> rules and regulations are used. <br /> 1.4 Permits and Local Approval <br /> 1.4.1 Wherever necessary,obtain approval from local authorities before beginning any work.Obtain <br /> any required permits for activities relating to the alteration or repair of tanks or equipment in connection <br /> with the storage, handling or use of regulated flammable or combustible liquids. <br /> 1.5 Personnel Training Requirements <br /> 1.5.1 Employees who perform repair or lining work shall be properly trained in the following: <br /> a) Handling and disposal of tank bottom sludge and residue. <br /> b) Use of equipment and procedures for testing and vapor-freeing tanks. <br /> c) All applicable safety rules and regulations. <br /> d) Handling and application of tank lining material. <br /> e) OSHA required permit for confined space entry requirements and procedures.All personnel <br /> participating in the tank lining process shall have been thoroughly trained in all aspects of <br /> confined space entry, and shall have completed an OSHA approved 40-hour course for <br /> personnel involved in confined space entry and procedure.See Appendix 1 for more detail <br /> on confined space classifications. <br /> Section 2 - Internal Lining and Repair of Steel Underground Storage Tanks <br /> 2.1 General Safety Requirements <br /> 2.1.1 Before excavating the tank access area,remove all sources of ignition from the area surrounding <br /> the tank and its vapor vents.Shutdown all open flame and spark producing equipment or machinery until <br /> the area is determined to be vapor-free.Use a combustible gas indicator to check and monitor hazardous <br /> vapors. Erect barricades and post signs reading "Flammable-No Smoking" around the affected area. <br /> 2/99 <br /> Page 3 of 30 <br />
