The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information:and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cocapit at(209)468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for Inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health &Safety Code. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses gi of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c.
