Date: June 25, 2015 The monitoring plan was not current when field verified for accuracy during the monitoring system certification testing. The monitoring plan needs the following corrections: TA0131016(diesel) needs to list the mechanical line leak detector manufacturer as Fe Petro and the model needs to be listed as STP MLD. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The UST site plan diesel tank leak detector listed should not be Red Jacket. It should be FE Petro STP MLD. The red diesel tank leak detector listed should not be VMI. It should be FE Petro STP MLD D. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the 10-16-14 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L7 91 stp sump 9-25-14 fuel alarm. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 314 CCR 2631(a) Primary containment not tight (pre -Jul 2003). Approximately a cup of diesel fuel was on each side of the diesel A B stp sump saddle, indicating a leak in the primary piping system. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a repeat violation, Class II.
