Immediately log <br />into CERS, make the necessary changes, and submit for review by the EHD. <br />This is a repeat violation, Class II. <br />107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />An accurate UST Monitoring Site Plan was not submitted. The UST site plan diesel tank leak detector listed should <br />not be Red Jacket. It should be FE Petro STP MLD. The red diesel tank leak detector listed should not be VMI. It <br />should be FE Petro STP MLD D. <br />A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the <br />California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST <br />Monitoring Site Plan. <br />This is a repeat violation, Class II. <br />207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the 10-16-14 designated <br />operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms <br />include: L7 91 stp sump 9-25-14 fuel alarm. During the monthly inspection, the designated operator shall review the <br />alarm history for the previous month, check that each alarm was documented and responded to appropriately, and <br />attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure <br />that designated operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />This is a repeat violation, Class II. <br />314 CCR 2631(a) Primary containment not tight (pre -Jul 2003). <br />Approximately a cup of diesel fuel was on each side of the diesel A B stp sump saddle, indicating a leak in the <br />primary piping system. All primary containment for the UST system must be tight.
