Lodi, CA 95240 SR0069996 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to install a Veeder Root 794380-208 sensor in the 87 turbine sump <br />on the underground storage tank (UST) system at the above noted address. This sensor was <br />replaced on 6-27-14 and functionality verified during the annual monitoring system certification <br />testing on 6-30-14. The updated scope of work includes installing a VMI 99 LD lead detector on <br />the 87, 91, and Diesel "A" turbines and installing a VR794390-409 sensor for the Red Diesel <br />UST. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Veloso at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. If it is determined that <br />a cold start is necessary, a copy of the alarm history shall be printed prior to <br />performing the "cold start" of the Veeder Root system and made available at the time <br />of EHD inspection. <br />b. Leak detector test: Leak detector must be tested to confirm that the device can <br />detect a leak of 3 gallons per hour at 10 psi. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />
