Lodi, CA Inspection <br />Name: Date: <br />Jun 25, 2012 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />109. Annual monitoring system certification, leak detector testing, and spill container testing were <br />performed on 6-27-11 and a test report was received after thirty days by the EHD on 8-2-11. A copy of <br />the test report must be submitted within 30 days of the tests. Correct immediately by taking all necessary <br />precautions to ensure that test reports are submitted within 30 days. <br />303. The diesel AB turbine sump sensor was located approximately three inches off the lowest point of <br />the sump and not located to detect a leak at the earliest opportunity. The red dye diesel and 87 fill sump <br />sensors were located approximately five inches off the lowest point of the sump and not located to <br />detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a <br />leak at the earliest possible opportunity. During the inspection, the technician relocated the sensors to <br />the lowest point of the sump to detect a leak at the earliest opportunity. Continue to ensure that sensors <br />are properly located to detect a leak at the earliest opportunity. Corrected on site. <br />311. The 91 and diesel B line leak detectors failed to detect a leak when tested. All line leak detectors <br />shall be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, <br />trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) <br />and obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br />immediately, there is a possibility that the 91 and diesel AB UST system may be red tagged to prevent <br />fuel inputs. <br />313. The diesel spill container failed when tested. All spill containers shall have a minimum capacity of <br />five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />contact a properly licensed, trained, and certified contractor to repair or replace the spill container under <br />permit and inspection of the EHD. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 7-25-12. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />V�miAeA 4Tea I Sit- <br />EHD 23-02 Rev 04/19/12 Page 3 `) UST INSPECTION REPORT <br />
