Lodi, CA 95242 SR0058876 <br />The permit application to install Blue Line SB Series Split Repair Boots on the 87 and 91 product <br />lines in the STP piping sumps and if needed install Blue Line SB Series Split Repair Boots at the <br />under dispenser containment sumps has been approved for the above noted address. <br />Conditions of approval: <br />1. Contact the local Fire Department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supercede <br />the previous ones. <br />3. Contact Aris Cacapit, REHS (209) 468-3453 to arrange the following inspections (a 48- <br />hour notice is required): <br />a. 87 and 91 line secondary containment tests. <br />b. 24 hour lake test or equivalent of the 87 and 91 STP sumps. <br />c. If needed 24 hour lake test or equivalent of the UDC sumps. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />
