All <br />of the violations that were documented during the November 5, 1998 routine inspection <br />were also noted on the November 23, 1999 re -inspection report. <br />Your file was reviewed on November 24, 1999 for required documentation and the <br />following additional violations were discovered: <br />1) The California Code of Regulations (CCR) section 2636 (f) (4) states that <br />"Monitoring shall be conducted on all underground piping systems with secondary <br />containment at least annually at a pressure designated by the equipment <br />manufacturer, provided that the method is capable of detecting a minimum release <br />equivalent to 0.1 gallon per hour defined at 150% of the normal operating pressure <br />of the product piping system at the test pressure with at least a 95% probability of <br />detection and not more than a 5% probability of false alarm." <br />Line testing results have not been submitted for 1995, 1996, 1997, 1998 and 1999. <br />2) CCR section 2630 (d) states "All monitoring equipment used to satisfy the <br />requirements of (CCR) sections 2632, 2634, and 2636 shall be installed, calibrated, <br />operated, and maintained in accordance with the manufacturer's instructions, <br />including routine maintenance and service checks (at least once per calendar year) for <br />operability or running condition. Written records shall be maintained as required in <br />CCR Section 2712 of Article 10." <br />Monitoring equipment testing results have not been submitted for 1996, 1997, 1998 <br />and 1999. <br />A Division of San Joaquin County Health Care Services <br />
