Attached to the form was the receipt from MB Services, dated <br /> 3/18/2016 with a hand written note "regarding Item#209, March 15 and 18, 2016". The RTC did include documentation <br /> to close violation 201, but did not address parts of violation 209, nor violations 306, and 315.
