The 87 electronic relay failed to provide positive shut down during the sensor out & failed
safe/ power out testing and when testing the L5 Unleaded 1 sump and L6 Unleaded 2 STP sump
sensors. Liquid detection sensors, when dipped into liquid for testing, were able to activate an audible
and visual alarm but the turbines continued to run. Possible electronic issues for the 81 electronic relay.
All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of
product at the dispenser when it detects a leak. Correct immediately by having a properly licensed,
trained, and certified contractor replace the failed component with a functional component (LG 113-x
listed, if applicable) and obtain a permit immediately from the EHD. If the failed component can't be
replaced immediately, there is a possibility that the 87 UST system may be red tagged to prevent fuel
inputs & outputs.
- Corrected on site. Outside service contractor called in for repairs. Contractor MB technician Byron
Melendez ICC 5261224UT exp 04/13 VR A27736 exp 02/14. replaced electronic relay & verified pos` ve
shutdown to 87 turbine during sensor functionality test
311. The 87 line leak detector failed to detect a leak when tested. Per testing technician, the 87 aur• ine
is in constant pressure mode and turbine always on since the Veeder-Root monitoring panel can no
longer operate the turbine. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak
at 10 psi. If the failed leak detector can't be replaced immediately, there is a possibility that thr- 87 UST
system may be red tagged to prevent fuel inputs & output.
- Since the 87 relay has been repaired, have a testing technician verified functionality of 87 line leak
detector. Submit a 48 hour advance notification to EHD to witness testing. Inspection will be billed at
current hourly rate.
Submit to SJC EHD within 30 days (1/7/13):
1. provide 48 hours advance notification of 87 line leak detector functionality testing to EHD
2. one copy of the completed "California Environmental Reporting System Lead User Authorization
Form" to allow EHD authorization to facility's CERS data. Businesses must submit electronic data of
HMBP on a state website starting 2013.
ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
R„ed gy Title: e
EHD 23-02 Rev 04/1 /12 Page 3 UST INSPECTION REPOF
