RADC Enterprises, Inc. sent documents overnight to SJC EHD Jan. 28, <br /> 2011. <br /> 13. Waiting on Permit to be Issue for the repair and Thuy Tran with SJC wants to be <br /> ON SITE when the repair is being done. <br /> 21. Corrected onsite at the time of Violation by Epic Technician placed into <br /> Hazardous Waste drum. <br /> 23. All Service Call documentation requested work order 07/21 and 08/08 sent to <br /> site on 2/1/2011 overnight, DiMaggio Technician will meet with Epic designated <br /> operator to Ensure all required information is on reports. EPIC has failed us with <br /> insuring that all alarms got documented and all test dates were written correctly <br /> therefore the only way I can insure this will not happen again is by replacingEPIC <br /> with a reputable Designated Operator. The change of DO will happen in the <br /> beginning of April 2011. <br /> 24.25. & 26. was completed on 1/5/2011 14 days late. Need to take all necessary <br /> precautions to ensure testing is performed in a timely manner. <br /> Submit to SJC by 2/7/2011 <br /> 1. Return to Compliance Certification form <br /> ( DiMaggio & Epic for Owner to Sign Documentation ) <br /> 2. A written corrective action statement ( DiMaggio & Epic ) <br /> 3.
