UST facility form on file with EHD is incomplete. On Facility Form A, the document is missing the BOE <br /> UST Storage Fee Account Number. Any change of information on these forms must be submitted to the <br /> EHD within 30 days of the changes. Immediately complete and submit a copy of the completed facility <br /> form to the EHD by 02/07/11. <br /> 4. Financial responsibility documents have not been submitted to the EHD for both 2010 and 2011 year. <br /> Facility had new owner/ operator around 07/2010. Current financial responsibility documents are <br /> required to be submitted annually. Complete and submit a copy of the financial responsibility <br /> (Certification of Financial Responsibility & Letter from Chief Financial Officer) for 2011 year by 02/07/11. <br /> 5. The monitoring plan on site are not current and not approved by the EHD. The monitoring plan must <br /> be current and approved by the EHD. Section V. the pipe monitoring liquid leak sensor is listed <br /> incorrectly. Comments section X. information is not correct. Complete and submit a copy of the <br /> monitoring plan for approval by the EHD by 02/07/11. An approved copy will be returned for your <br /> records. <br /> 13. The 91 STP sump sensor failed to activate an audible and visual alarm when tested to ensure <br /> continuous monitoring of the tank system. Per technician, sensor resistance reading were high which <br /> did not allow the audible and visual alarm to trigger. <br /> The 87 STP sump sensor failed to provide positive shut down to the turbine when tested in liquid. <br /> Although it produced an audible and visual alarm, the relay contact was stuck & turbine didn't shut off. <br /> This monitoring equipment is programmed to activate an audible and visual alarm and stop the flow <br /> of product at the turbine when it detects a leak. Correct immediately by having a properly licensed, <br /> trained, and certified contractor replace the failed component with a functional component (LG 113-x <br /> listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component <br /> can't be replaced immediately, there is a possibility that the 91 and 87 UST system may be red tagged <br /> to prevent fuel inputs. <br /> 21. Approximately 3 gallons of liquid was observed in the 91 fill sump. If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br /> regulations, and manage it accordingly. Ensure that the 91 fill sump is maintained free of liquid. <br /> Corrected onsite.
