UST facility form on file with the EHD are not current and lacking the tank form. Any change of <br />information on these forms must be submitted to the EHD within 30 days of the changes. Immediately <br />complete and submit a copy of the facility and tank forms to the EHD within 30 days of receiving this <br />report. Complete a separate tank form for each tank. <br />3. UST facility form BOE number is incorrect. Immediately complete and submit a copy of the facility <br />form to the EHD within 30 days of receiving this report. <br />6. Current approved copy of the monitoring plan was not found on site. Copy of this plan shall be <br />retained on site at all times. Immediately locate and retain a copy on site. <br />21. Liquid was observed in the 87, 91, and diesel turbine sumps. If water could enter into the <br />secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br />Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br />regulations, and manage it accordingly. Ensure that the87, 91, and diesel sumps are maintained free of <br />liquid. The sumps were cleaned by the technician and the waste generated was put in the facility's <br />hazardous waste drum. <br />23c. The designated operator failed to check and note the correct test dates for the secondary <br />containment test date on 7/13/11 designated operator monthly inspection report. The correct test date is <br />6/9/11 instead of 6/9/08. During the monthly inspection, the designated operator shall check that all <br />required testing and maintenance for the UST system have been completed and document the dates <br />they were done. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />14. A fuel leak was found at the test port on the shear valve of the 91 product line. All primary <br />containment must be product tight. Immediately discontinue use of failed piping and have a properly <br />licensed, trained, and certified contractor replace or repair the failed piping under permit and inspection <br />of the EHD. If the failed component can't be replaced immediately, there is a possibility that the 91 UST <br />system may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: I <br />110 / V19-'01, <br />Received By: <br />Title: <br />>� <br />EHD 23-02-003 Rev 06/01/10 CONTINUATION FORM <br />
