UST facility form on file with the EHD are not current and lacking the tank form. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to the EHD within 30 days of receiving this report. Complete a separate tank form for each tank. 3. UST facility form BOE number is incorrect. Immediately complete and submit a copy of the facility form to the EHD within 30 days of receiving this report. 6. Current approved copy of the monitoring plan was not found on site. Copy of this plan shall be retained on site at all times. Immediately locate and retain a copy on site. 21. Liquid was observed in the 87, 91, and diesel turbine sumps. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the87, 91, and diesel sumps are maintained free of liquid. The sumps were cleaned by the technician and the waste generated was put in the facility's hazardous waste drum. 23c. The designated operator failed to check and note the correct test dates for the secondary containment test date on 7/13/11 designated operator monthly inspection report. The correct test date is 6/9/11 instead of 6/9/08. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 14. A fuel leak was found at the test port on the shear valve of the 91 product line. All primary containment must be product tight. Immediately discontinue use of failed piping and have a properly licensed, trained, and certified contractor replace or repair the failed piping under permit and inspection of the EHD. If the failed component can't be replaced immediately, there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115).
