An accurate UST Monitoring Site Plan was not submitted. The site map does not include the sensors that monitor the UST system. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The 2014 Secondary Containment test results and the 2012 Monitoring System Certification test results were not found onsite. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. The daily alarm log did not document all alarms from the alarm history. The alarms not documented include: - L4: Diesel STP Sump December 25, 2014 - L4: Diesel STP Sump December 2, 2014 All alarms from the alarm history need to be documented and made available for review by the EHD upon request. Immediately began documenting all alarms from the alarm history and have available for review by the EDH upon request. This is a repeat violation, Class II. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II. 319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). Liquid was observed in the 91 and Diesel STP sumps . If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed 1 gallon of liquid from the 91 STP sump and 1/2 gallon of liquid from the Diesel sump. Ensure that all sumps and annular spaces are maintained free of liquid.
