Hassan Ahmed/Site Manager confirmed that there was a power <br />surge/glitch around early July as a vehicle hit the electric pole that may have caused the tank <br />monitor to lose data. (Please see attached email from LC Services/Hope) <br />(If we need to do Monitoring Re -certification please let me know) <br />Karen Arnaiz/Designated Operator have corrected the testing dates showing on her <br />monthly report. (Please see attached letter from Karen) <br />Resolution Class II Violation # 205: <br />Missing Designated Operator monthly inspection dated December 2013 and May 2014 has <br />been provided to the County. <br />MINOR- Notice to Comply: 105, 106, 109, 999 <br />Resolution Minor -NTC # 105 & 1.06: <br />UST Tank Information forms for tanks #: TA0507895 and TA0507897 has been corrected <br />and submitted thru CERS on August 8, 2014. <br />Annual Monitoring Plan for tank #: TA0507896 has been corrected and submitted thru <br />CERS on August 8, 2014. <br />Response Plan was submitted thru CERS on April 1, 2014 and accepted on June 27, 2014. <br />Per LC Services/Hope tech left a copy of Annual Monitoring System Certification test <br />result at the site on the day of the testing 8/8/2013 and submitted a copy of the same test result to <br />Environmental Health Depart on August 8, 2013 within 30 days of testing. (Please see email <br />attached) <br />All required paperwork has been uploaded and submitted thru CERS on August 8, 2014 <br />9/15/2014 <br />
