A checklist of findings was left onsite <br />at the time of inspection. This Is a revised report issued on 8-7-2014 and it supersedes all <br />previously issued reports. <br />Class 11 Violations: 117, 201, 204, 205 <br />117. Based on the lack of alarm history at the time of inspection, an apparent cold start was conducted <br />at this facility and a permit was not obtained from the EHD. An alarm history was included on the <br />designated operator report dated 6-10-2014, however, it was was not available on the Veeder Root print <br />out included on the designated operator report dated 7-15-2014. Emergency repairs relating to UST <br />system monitoring and release detection may be performed by a properly licensed, trained, and certified <br />contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. <br />All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. <br />Immediately obtain a permit from the EHD for the conducted cold start. A "Failure to Obtain a Permit" <br />penalty applies. Since all pertinent sensors were successfully tested during the recent monitoring system <br />certification, no follow-up inspection is required at this time. <br />201. No maintenance records for the apparent cold start, which took place between the 6-10-2014 and <br />7-15-2014 were found on site. These records shall be kept on site for at least 3 years. Immediately <br />locate <br />and maintain legible maintenance records on site and submit copies along with the corrective <br />action statement to the EHD by September 7, 2014. <br />204. The designated operator failed to check and note the correct test date for the monitoring system, <br />certification on all designated operator inspections reports for the last 12 months (August 13, 2013 <br />through July 15, 2014). The test date 7-8-2013 instead <br />was noted as of 8-8-2013. During the mo y <br />inspection, the designated operator shall check that all required testing and maintenance for the UST <br />system have been completed and document the dates they were done. Ensure that designated <br />operators performing monthly inspections at this facility are including all of the required information on <br />the reports, <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY 13 SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD 23-02 Rev 04!19112 <br />.... ...... <br />Page 3 UST INSPECTION REPORT <br />
