Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on July 31, 201SLand the test report has not been submitted to the EHD within the required <br />time frame. A copy of the test report must be submitted within 30 days of testing to the EHD. Ensure <br />that future reports are submitted to the EHD within the required time frame. <br />305. The Bravo box float and chain in the UDC 1/2 and UDC 3/4 sump failed to stop the flow of product <br />at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br />adjusted the chains and verified functionality. <br />313. The 87 -octane direct -bury spill container failed when tested. All spill containers shall have a <br />minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or <br />cleaned up. Immediately discontinue deposition of petroleum into this tank until the component is <br />repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection <br />of the EHD. If the spill container can't be replaced immediately, there is a possibility that the 87 -octane <br />UST system may be red tagged to prevent fuel inputs. <br />Minor -Notice to Comply: (105) <br />105. UST Facility Operating Permit Application and UST Tank Information for all existing site tanks are <br />not current in CERS; no overfill prevention method is identified. Immediately log into CERS, update the <br />required information, and submit for review by the EHD.
